Dr. Douglas Layton, DO, FAAFP
Dr. Layton is board certified in family medicine and has 25 years of experience. His undergraduate degree is from Simpson College. He went to medical school at the University of Osteopathic Medicine and Health Sciences in Des Moines. He completed his residency at North Iowa Mercy Medical Center in Mason City, IA. He has earned the title of Fellow in the American Academy of Family Physicians. He loves the diversity of family medicine and is very passionate about preventative medicine. His hobbies include hunting and fishing.
Jennifer Blaylock ARNP
Jennifer is a board certified nurse practitioner with 8 years of nursing experience. Jennifer graduated from Old Dominion University in Norfolk, VA with her Bachelor’s of Science in Nursing and received her Master’s of Science in Nursing at Chamberlain University in 2022. Jennifer started her nursing career in the emergency department at Iowa Methodist Medical Center in 2015 and transferred to Life Flight nursing from 2019-2021. Jennifer began working at North Polk Family Medicine 2 years ago as a registered nurse and is excited to transition to her new role. Along with providing primary care services Jennifer attended Aesthetic Medical Educators Training to diversify her skill set to be able to offer medical aesthetic services to the community and surrounding areas. Jennifer has lived in Polk City for 6 years with her husband Riley and their son Nolan. Jennifer’s hobbies include camping, golfing, and going to the lake.
